Certifying the sameness of person

Confirmation of identity of an individual with the person depicted in the photograph.

1. The notary shall certify the identity of a natural person with the person depicted in the photograph by issuing a respective certificate. When certifying the identity of the person depicted in the photograph with a minor, the rule set forth in paragraph 3 of Article 19 of this Instruction shall be applied.
2. The certificate shall obligatory contain the following information: name and surname of the person depicted in the photograph, date and place of birth, sex, place of residence, as well as details of the document on the basis of which the identity of the person depicted in the photograph is established. The photograph shall be placed in the upper left corner of the certificate and shall bear the seal of the notary in such a way that part of the seal is reflected in the lower right part of the photograph and part on the sheet on which the photograph is placed.
